Abstract: When patients express a wish to die, it can often be misinterpreted as a desire to hasten death or suicidal ideation. However, a wish to die is an expression that can fluctuate throughout a serious illness and need not imply either. As care providers, we must be careful that our initial reactions are not focused on fixing, fulfilling, nor dismissing this request. Instead, providing therapeutic presence and exploring the meaning behind these requests can reveal supportive and individualized interventions.

Abstract: Pennsylvania Orders for Life-Sustaining Treatment (POLST) plays a critical role in translating patient preferences into actionable medical orders across care settings. Yet, despite its widespread use, misconceptions about POLST persist, and challenges often arise during care transitions and clinical conversations.
This interactive presentation explores POLST in real-world practice, focusing on common myths and misunderstandings that can lead to inappropriate use or misinterpretation. Participants will examine the complexities of initiating, revisiting, and honoring POLST orders, particularly during transitions across care settings.
Through exploring best practices and delivering practical guidance, this session equips clinicians with tools to ensure POLST is used appropriately, aligned with patient goals, and integrated into high-quality, patient-centered care.
